The Chapter 5 Season 4 NPC roster includes 4 new faces: Captain Jonesy, Peelverine, War Machine, and Hope has been remixed with a new Heroic Hope version. Meanwhile, the other 9 characters on the island are back after appearing in previous updates.

The Wasteland Warrior NPC bosses in Season 4 drop their own new medallions, which contain their own unique abilities and mythic weapons.
Here are all the boss locations where they spawn and the mythic weapons and items they drop when defeated:
- Doombot – Spawns at Castle Doom (drops Siphon Medallion, Doctor Doom’s Mythic Arcane Gauntlets, and Mythic Monarch Pistol)
- Mysterio – Found in Doomstadt (drops the Stealth Medallion and the Sovereign Mythic Shotgun)
- Emma Frost – Spawns at the Raft (drops Mythic Revelation Medallion and Striker Burst AR)
